Contractor cited in death of Westfield Police Officer Jose Torres

WESTFIELD, Mass. (WWLP) - The construction company involved in the death of Westfield police officer Jose Torres has been cited by OSHA. 

An OSHA representative told 22News their on-site investigation is complete. Torres died on July 26, 2012 after being struck by a dump truck, while working a construction detail on Pontoosic Road.

Revoli Construction Company, based out of Franklin, Massachusetts, has been cited in the tragic accident. 

22News received a copy of the citation sent to Revoli Construction Company.  This is a repeat violation, since the company was also cited for an incident in November 2009 which happened in Needham, MA.

The results of OSHA's investigation claim that Revoli Construction Company "did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury."

Also, "On or about July 26th, 2012, Revoli Construction did not instruct other employer's employees on the hazards created when Revoli employees moved vehicles around this job site."

The proposed penalty is $14,000.  The company has 15 days to have an informal discussion with OSHA that could lead to a settlement or they can also contest this citation within 15 days.
